Some benefits have changed for the new plan year. Below are some items to review for 2024:

MEDICAL
BrightView will continue to offer two medical plan options through BPA with the Cigna PPO network. Effective 1/1/24, BrightView will also offer a Health Reimbursement Account through Garner Health to help you REDUCE your deductible and out-of-pocket maximums!
Garner equips you to find the best doctors for your specific needs! Once selecting an approved provider, you will be eligible for reimbursement for services received from that provider for up to $2,000 if enrolled in individual coverage or up to $4,000 if enrolled in family coverage. This reduces your deductible liability as follows:
Value Based PPO
Plan Deductible: $2,500 single/$5,000 family
*After Garner Reimbursement: $500 single/$1,000 family
Simple Savings Plan
Plan Deductible: $5,000 single/$10,000 family
*After Garner Reimbursement: $3,000 single/$6,000 family
*Requires use of Garner "Top Provider"

EVIDENCE OF INSURABILITY (EOI) FORMS

As a reminder, EOI forms are required under the following circumstances:

LTD and STD Coverage:

If you are electing coverage mid-year through a qualifying event

If you have a previously declined EOI on file with Lincoln

2024: if you are electing coverage over the guarantee issue

If you are newly electing coverage after previously waiving coverage
If you are increasing your current coverage by more than 2 increments

DEPENDENT ELIGIBILITY
BrightView, LLC extends group coverage to your spouse/domestic partner and children. In order to enroll new dependents under a policy, an employee will be asked to provide documentation testifying that each covered person meets the criteria of a qualified dependent set forth by the ACA.

SPOUSAL CARVEOUT
In order to keep your premiums low, BrightView, LLC’s health plan will not cover an employee’s spouse if the spouse is offered coverage through his or her employer. Limiting spousal coverage to those who do not have other opportunities to obtain group coverage helps ensure that we can continue to offer you a quality health plan with small premiums.
An affidavit will be required to attest your spouse’s eligibility if spousal coverage is elected. Download a copy of the affidavit HERE
